Patient Rights
ADMISSION INTO SEABROOK HOUSE FOR CHEMICAL DEPENDENCY DOES
NOT CONSTITUTE THE SURRENDER OF YOUR RIGHTS AS A HUMAN BEING. SEABROOK
HOUSE SUPPORTS AND PROTECTS THE FUNDAMENTAL HUMAN, CIVIL, CONSTITUTIONAL
AND STATUTORY RIGHTS OF EACH PATIENT. WE WANT YOU TO BE INVOLVED
IN YOUR CARE, AND AS SUCH, YOU, AS THE PATIENT, AND/OR YOUR FAMILY
(PARENTS/GUARDIANS) HAVE THE RIGHT TO:
- Reasonable access to treatment, adequate and humane services regardless of your race, religion, sex, sexual orientation, ethnicity, age, handicap, or financial status.
- Provision of services within the least restrictive environment possible that preserve personal dignity.
- Communication regarding your assessment, diagnoses and treatment options.
- Participate in the development of an individualized treatment plan, which is reviewed regularly by the treatment team, as well as communication regarding changes to your plan or the staff that work with you, regardless of your source of financial support.
- Care that is considerate respects your personal value/belief system, by an adequate number of competent, qualified, and experienced professional clinical staff.
- Freedom from censorship of mail or phone calls, unless therapeutically contraindicated.
- Assurance of your health and safety. Emergency medical treatment is arranged for and transportation provided, but Seabrook House is not responsible for patient's medical bills if they are hurt or require medical attention while in treatment. We will provide medical treatment in relation to the diagnosis under which you were admitted.
- Information/participation regarding the nature of your care, procedures and treatments you will receive, including any side-effects, risks, and benefits of all medications and treatments, alternative treatments available (transfers), and your right to refuse medications and treatments.
- Freedom from requirement to perform tasks that may cause injury or emotional trauma. A part of treatment is personal care, which includes making your bed, dusting your area, and clearing your plates from the dining room. However, there is no physical labor at Seabrook House, nor do patients perform any work other than those for personal care.
- Request the opinion of a consultant, at your own expense, or an in-house review of your treatment plan.
- Visits from family and significant others, regardless of age, unless contraindicated.
- Obtain the costs, as appropriate, itemized whenever possible, of services rendered, the source of our reimbursement and any limitations placed on the duration of services.
- Rules and regulations of Seabrook House that govern your conduct during your stay with us. Please refer to the statement of Patient Responsibilities.
- Confidentiality and privacy as outlined in your "Patient Standards."
- Plans for meeting and continuing mental and physical health requirements following discharge.
- To express your personal values, belief systems, and cultural practices. You are encouraged to discuss it with clinical/medical staff. However, these beliefs and practices may not harm others or interfere with the planned course of action.
- Leave treatment at your own responsibility. If at any time you desire to leave, please discuss this decision with staff so that appropriate procedures can be followed and we can arrange for safe transportation to your home. If you are an adolescent and leave treatment without Seabrook House arranging for your parent/legal guardian to pick you up, your parents/legal guardians will be called, as well as the New Jersey State Police.
- If during your stay, staff makes a judgment that you pose as a threat to yourself or others, it is our responsibility to seek appropriate legal action. This will include the involvement of the police or orders for involuntary treatment at another facility. If you are an adolescent, your parent/guardian will be called, as well as the New Jersey State Police.
- You have the right to participate in consideration of ethical issues, including patient grievance, surrogate decision-makers, withholding resuscitation and life preserving treatments (advance directives), and investigational studies or clinical trials.
- The right to file a complaint or grievance either orally, written or through established Seabrook House procedure. If complaint or grievance is not satisfied through Seabrook House procedures, you may file a complaint with the State of New Jersey at 1-800-792-9770 help line number.
The Medical Director, and the Vice President of Treatment Services are primarily responsible for all care and services provided at all locations. This includes authorizing procedures and treatments to be used by professional staff during your treatment at Seabrook House.
The only staff at Seabrook House that may perform any medical procedure or treatment, are physicians and nurses. In addition to the Medical Director, who is a licensed physician and certified in Addiction Medicine, Seabrook House has a consulting agreement with and other area physicians. These physicians are licensed physicians with the State of New Jersey, and also certified in Addiction Medicine. Should you see any other physician from the practice, our nursing staff will identify them. Seabrook House nursing staff consists of LPNs and RNs, all licensed by the State of New Jersey. These individuals are primarily responsible for your medical care during your treatment. Their identity is evidenced by their Employee Identification Badges, which includes their picture, title, and department.
Members of Seabrook House's professional staff provide clinical procedures and treatments. If you are being admitted to an inpatient program, the identity of the professional staff member who will be primarily responsible for performing/directing these procedures/treatments, will be named on your Patient Identification Bracelet within 24 hours of your admission.
Anyone in our care, who experiences an after-hour emergency of a clinical nature, can contact Seabrook House at 1-800-582-5968. The staff member on duty will assess the situation, and, as appropriate, take action or contact appropriate clinical staff available on beeper.
In addition to members of the "Professional Staff", Seabrook House also employs Consulting Professional Staff. The Consulting Professional staff consists of a consulting psychiatrist, a consulting psychologist, and a consulting dietitian. The consulting dietitian reviews all records; the Consulting psychiatrist and psychologist are only involved in your care when called upon by a member of the Active Professional Staff. The staff will inform the consultant of the specific reason(s) why you are being referred, and they provide our treatment team with specific recommendations and/or answers to questions, to help our staff provide the necessary interventions.
